• Partner One (1) listed on the civil union license.
• Partner Two (2) listed on the civil union license.
• Immediate family of partner one (1) or partner two (2).
• Those with written, notarized consent from the individuals.

Civil Union Certificates are copies of a civil union license used to prove a civil union.

To purchase a Certified Copy of a Civil Union Record, you will need the following:

• Partner One's (1) full name, at the time of the civil union.
• Partner Two's (2) full name, at the time of the civil union.
• The Month, Day and Year of the Civil Union.
• The City that the Civil Union took place in.
• Provide a valid photo ID such as a Driver's License or State ID.
• The fee is $15.00 for the first Certified Copy and $8.00 for each additional copy of same record purchased at the same time.
